Job Overview

An opportunity has risen for an enthusiastic Band 6 nurse with cardiac nursing experience to join the Heart Failure inpatient and community service with ABUHB. The service provides care to people living with heart failure across the Health Board, and provides face to face and remote assessments. Clinics are held at Ysbyty Aneurin Bevan, Ysbyty Ystrad Mynach, County, Chepstow, Newport, Monmouth and Nevill Hall Hospital.

The role provides involves supporting the existing Band 7's, with case management both in the inpatient and community setting.

The service is committed to providing an evidence, value based service that embeds Patient reported outcomes into care.

We are looking for someone who has these values and embraces change and service development.

Main duties of the job

To develop professionally and assist in providing an integrated/sustainable heart failure (HF) service.

To provide care for patients in a holistic manner incorporating physical, and psychological care with social consideration to enable them to live with Heart Failure.

To manage a case load of patients within a multi professional team, ensuring that patients with HF of different aetiologies and treatment pathways receive the highest standards of care and support from the time of diagnosis through the course of their disease trajectory.

Demonstrate excellent clinical assessment skills with a view to optimising patient’s medication.

To provide professional support, clinical expertise and appropriate education and training to all those professionals involved in providing care to patients our patients.

To assist in leading and developing the Heart Failure service and palliative care, to meet the requirements of NICE guidelines ensuring a continuous process of evaluation.

Undertake an active role in promoting and carrying out research in conjunction with other health professionals – seeking to have work published.

The ability to speak Welsh is desirable for this post; Welsh and/or English speakers are equally welcome to apply.

Experience

Essential criteria

  • First Level Nurse – RGN/RN
  • Graduate
  • Post registration cardiac development
  • Experience of working with cardiac patients
  • Experience of managing own case load

Desirable criteria

  • Experience of health promotion
  • Experience of heart failure patients
  • Masters degree or working towards
  • Prescribing experience
  • Teaching experience
