Community Staff Nurse

The post holder will be responsible for ensuring the highest standard of clinical care is provided for patients referred to the team for District Nursing Services.

The role focuses on autonomous clinical practice, patient assessment, care coordination, and delivery of complex care in the community, ensuring patients receive safe, effective care in their own home.

The post holder must adhere at all times to the Code of Conduct as published by the Nursing and Midwifery Council and must work within the policies and guidelines as laid down by Airedale NHS Foundation Trust.

Clinical Care

  • Deliver high-quality, evidence-based nursing care and clinical procedures to patients in their own homes or community settings
  • Undertake holistic patient assessments, including physical, psychological,and social needs
  • Plan, implement, and evaluate individualised care plans
  • Maintain accurate, timely documentation using electronic patient record.

Assessment and Care Planning

  • Undertake initial and ongoing patient assessments to identify care needs and risks.
  • Recognise deterioration and escalate concerns appropriately.

Caseload Support

  • Support the management of the caseload under the supervision of a District Nurse
  • Prioritise workload and escalate any concerns.
  • Work collaboratively with members of the MDT.

Patient and Carer Support

  • Provide education and advice to patients, families, and carers to promote independence and self-management
  • Support end-of-life and palliative care in partnership with specialist teams

Safeguarding and Risk Management

  • Identify and respond to safeguarding concerns in line with local and national policies
  • Assess and manage risks within the home environment

Professional Responsibilities

  • Maintain NMC registration and adhere to the NMC Code
  • Participate in clinical supervision, training, and continuing professional development
  • Comply with local and national policies
