We are looking for a Social Worker to work within Adult Social Care, in our West Community Hospital and Discharge to Assess Service. We are looking for someone who is enthusiastic and embraces a person-centred approach to practice. You will be part of a supportive team working within Adult Social Care operations.

The Role

You will be working as part of a team of Social Workers and Social Care Practitioners to work as part of the wider team to support discharge from Cornwall’s Community Hospitals. This role also includes working with individuals in discharge to assess placements. As a Social Worker you will complete timely, high quality and person-centred needs assessments under the Care Act 2014, alongside supporting to improve outcomes and increase/maintain the independence of adults requiring support from Adult Social Care.

This is a public/customer-facing role, where the statutory English language requirement for public sector workers applies.

This position will be subject to an enhanced criminal record disclosure check.

Working Pattern

This role is a full time post working 37 hours per week. There is opportunity for hybrid working as part of this role.

What You’ll Need To Succeed

  • To hold a recognised Social Work qualification (Dip.SW) or equivalent.
  • Be registered to practice as a Social Worker with Social Work England.
  • Expert knowledge and experience of relevant legislation, social work theory, policy and principles and evidence of these within your practice.
  • Previous experience of working in a social care environment.
  • Ability to undertake, analyse and translate complex assessments of need, risks and strengths.
  • Ability to form strong professional relationships with other partners within a multi-disciplinary setting.
