Outreach Support Worker

Starting salary £12.71ph
Rising to £12.91 after successful completion of probation, increasing to £13.45 after a further 12 months in post.
Our Outreach Service provides 1:1 support to people experiencing mental health needs, Substance use, Learning disabilities and or Autism who live in the community, as well as people in hospital or care settings who are moving back into the community, and require support to build networks prior to returning.
Our Outreach team requires community travel so driving licence is desirable but not essential.
We provide Person Centred support, so flexibility is essential to meet each individuals, needs, this may include working occasional evenings to support with activities.

Providing person-centred support within the community
Supporting individuals transitioning from hospital to home or supported placements
Assisting individuals experiencing homelessness with housing, routines, and accessing services
Supporting people to attend appointments, manage daily tasks, and stay safe
Building trust with individuals who may have experienced trauma or instability
Lone working in the community following robust safety procedures.
Completing risk assessments, support plans, and accurate documentation

As an outreach Support Worker, you will provide high quality 1:1 person centred care and support to our clients to enable them to live their best life. You will be passionate in making a difference to our client’s lives, helping them to fulfil their aspirations and ambitions and live as independent as possible.
Skills & Experience Needed
Experience in community support, mental health, or homelessness services
Strong lone-working confidence and decision-making skills
Consistent professionalism and strong boundary management
Ability to adapt communication to diverse needs
Problem-solving skills and the ability to work independently
Empathy, resilience, and a motivational approach
Strong safeguarding knowledge and the ability to respond appropriately to risk
