We are seeking an enthusiastic and adaptable Community Nurse Team Leader to join our supportive and progressive team in Herne Bay. You will embody the Trust’s CARE values and work within a close-knit team dedicated to delivering high-quality, patient-centred care to the local community.

Our Community Adult Nursing teams are working closely with Primary Care Networks to drive innovative, holistic, and needs-led approaches to patient care. This is an exciting time to join us and be part of shaping the future of community services.

You will play a key role within a multidisciplinary team, managing patients with complex needs in the community. This role offers the autonomy to plan and deliver care, while developing your assessment and treatment skills with the support of strong clinical supervision and leadership.

You will also support junior colleagues in their development, helping to build a skilled and confident workforce for the future. For Community Nursing roles, a Specialist Practitioner (District Nursing) qualification is essential, although development and secondment opportunities may be considered.
