Advanced Clinical Practitioner

Working Full time: 37.5 hours per week

The post holder will be based on Poppy Ward at Ellesmere Port Hospital.

The post holder will be responsible for contributing to the ongoing development of clinical practice, research, and standards of care within the service, including development of policies and guidelines and contribute to service development to ensure patients have a seamless pathway.

The post holder may be required to work on other wards/ departments within the Trust at short notice in the event of hospital pressures/major civilian disaster.

The post-holder will be required to work within Poppy Ward at Ellesmere Port Hospital. The role entails assessing, treating and directing patients to the appropriate setting,

The Trust’s services are provided from three locations:

• The Countess of Chester Hospital: providing 473 general and acute beds.
• Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.
• Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
