Rainbow Care Solutions is a long‑established, CQC‑registered home care provider with over 20 years’ experience. We’re recruiting Full-Time and Part time hours ) to support our Discharge to Assess (D2A) / Reablement service across Herefordshire.

This role supports people returning home from hospital to recover safely and regain independence , doing with, not doing for.

Location

Herefordshire (community-based)

What you’ll be doing

Supporting people at home after discharge with reablement-focused care

Assisting with personal care where needed, while encouraging independence

Supporting daily living skills (washing, dressing, meal prep, mobility, routines)

Following care plans and recording notes accurately

Reporting changes in condition, concerns, or risks promptly

Working respectfully with families and professionals as part of a wider team

• A full UK driver's license and access to a vehicle.
