Role Purpose

Your Health Dudley (Integrated Health Improvement Service)

ABL Health delivers the Integrated Health Improvement Service, Your Health Dudley, commissioned by the Dudley Council – Public Health Team. This transformative service will play a central role in improving the health and well-being of residents of Dudley. The service provides integrated behaviour support to help people move more, drink less alcohol, stop smoking and lose weight. The service also offers lifestyle support for families, deliver community health checks, offers advice and guidance, and proactively signposts to relevant organisations. All of this is embedded in sustainable behaviour change and positive well-being. This post will focus on the Healthy Lifestyle support for families with children aged 4-17.

Key Duties and Responsibilities

As a Family Healthy Lifestyle Practitioner, you will work closely with children, young people and families to improve health and wellbeing through personalised support and evidence-based behaviour change interventions. You will provide one-to-one support via face-to-face appointments, telephone and video consultations, helping families to set achievable SMART goals and sustain healthy lifestyle changes. Working collaboratively with schools, community organisations, commissioners and local partner agencies, you will build strong referral pathways, promote the service, and increase engagement while reducing drop-out rates. You will design and deliver engaging workshops on topics such as healthy eating and active lifestyles, contribute to the development of family-friendly resources and social media content, and gather client feedback, case studies and testimonials to support service improvement. The role also involves maintaining accurate records and performance data, contributing to KPI achievement, safeguarding children and families in line with organisational policies, supporting the wider integrated service, attending meetings and training, and working flexibly to meet the evolving needs of the service.

Skills and Competencies Required

  • Be an experienced health improvement practitioner with a knowledge of working with families
  • Experience of evolving and developing services
  • Good team player with a positive outlook willing to engage and support others to achieve the highest standards and outcomes.
  • Adept in communication; A strong communicator who adapts the message to fit the needs of the audience including clients, staff, and partners
  • Able to build strong relationships, foster trust and co-operation among colleagues, stakeholders and throughout community settings
  • Be quality driven; naturally seeking high standards and looking for ways to achieve this
  • Value and remain open to new ideas and perspectives.
