Older Adult Home Treatment Practitioner

We are looking for an enthusiastic, compassionate, and experienced mental health practitioner to join our friendly and supportive Home Treatment Team for Older People in Rotherham.
This is an exciting post which has been funded specifically to support our emerging pathway for older people. The post holder will be responsible for providing support to patients later life working with a broad range of service providers across all tiers of Mental Health Care and with partner agencies; to deliver effective interventions that meet the needs of this population. The positions require a high level of autonomous practice, and practitioners will be confident clinicians able to make competent decisions about risk, care planning and the appropriateness of home treatment.
Postholders will have relevant knowledge and experience of mental health acute/ crisis work in the community. Applicants from a range of professions, including nursing, occupational therapy and social work are welcomed
This is an exciting time to work in Rotherham Urgent care services. We comprise of Crisis, Home Treatment and Hospital Liaison. We have strong links with the inpatient units, and the Locality teams. We also work closely with Partner agencies and third sector organisations to ensure we provide a comprehensive service to our community.
There are exciting developments within the Home Treatment team, as we work towards accreditation and expanding our offer to include Older People.
We are looking for passionate, reliable and progressive staff who work within a strengths and recovery based approach.
Liaison with families and supporting their understanding of crisis management is an important part of the role.
The post will require individuals to work alongside partner organisations, third sector and peer support services, transparency and openness is vital.
You must have a qualifications in Mental Health Nursing, Social Worker or Occupational Therapist. You must be registered with relevant governing bodies.
