KSS Community Integration Emergency Medicine Fellow
University Hospitals Sussex NHS Foundation Trust
This exciting new fellow post has been developed to align with NHS England’s Urgent and Emergency Care Plan to move care closer to home and reduce avoidable admissions.
Our Emergency Departments are facing unprecedented pressure with high demand and overcrowding. Our patients are increasingly complex, frail and multi morbid making it a necessity that we address challenges with the interface between primary and secondary care. The fellowship is designed to improve patient experience for frail, complex and vulnerable patients and reduce avoidable admissions. The successful applicant would be able to work with stakeholders to create sustainable, safe community pathways that offer evidence-based care for patients, especially those more vulnerable and frail patients where a visit to the emergency department can be extremely challenging. The EM resident recruited to this role would be able to gain experience in service delivery, management and developing leadership skills in dedicated time. The fellow will be able to lead on developing new service models and clinical governance for community care which will prepare them for senior leadership roles as consultant leaders of the future.
The Community Integration fellow is a split role: 40% of time spent working clinically in the Emergency Department and 60% of time (3 days a week) dedicated to fellowship activity.
The clinical duties of the post will be shop floor Emergency Department work, with opportunities to work in all areas of the department. Night shifts will be pro-rata for the clinical / non-clinical split, and weekend frequency a maximum of 1:3.
A considerable amount (3 days) of weekday time will be spent on fellowship activity, which will be planned alongside your supervisor and key stakeholders at the start of the placement.
Activities Will Include
- Personal development of quality improvement and change management skills.
- Identification of specific patient groups or presentations that might be better served through care pathways that avoid Emergency Department attendance.
- Collaboration with community stakeholders in general practice, local ambulance services, care homes to support their management of patients outside of the hospital setting.
- Local development of projects which deliver improved community-based care for patients, with safe and structured care pathways which result in reduction in certain presentations to the emergency department because of re-routing to other services or being seen and treated in the community.
At UHSussex, diversity is our strength, and we want you to feel included to help us always deliver Excellent Care Everywhere, as shown in our Outstanding for Caring CQC rating.
Your uniqueness and experiences will be part of our creative and innovative community where everyone is encouraged to succeed. We have a range of staff networks to help break down barriers, and can offer a buddy to help new members settle in.
