This is an exciting job opportunity to work within the Same Day Emergency Care therapy Team (SDEC).
Undertake physiotherapy assessment and intervention within the Same Day Emergency Care (SDEC) service and the Emergency Department (ED), managing a designated caseload of patients presenting with a range of acute conditions. The role focuses on rapid assessment, early intervention, and supporting safe same-day discharge by facilitating appropriate onward care within community services, including Community Response Hubs.
The post holder will provide a high-quality, effective physiotherapy service to patients presenting to SDEC and ED, ensuring timely assessment and treatment to support patient flow, prevent unnecessary admission, and promote safe discharge pathways.
This post is to cover a 7-day 8am-6pm service.
This post involves Respiratory Chest Physio with some on call.
Main Duties As Outlined Below
The post holder will work as part of the multidisciplinary team within SDEC and ED, providing specialist physiotherapy assessment to determine functional ability, mobility status and rehabilitation needs. They will develop and implement treatment plans and facilitate safe discharge planning by liaising with community services, including Community Response Hubs, to ensure patients receive ongoing support and rehabilitation where required.
The successful candidate will need to have had experience in the core Physiotherapy rotations in addition to being confident and competent in discharge planning and autonomous working within the Community setting.
The Trust’s services are provided from three locations:
- The Countess of Chester Hospital: providing 473 general and acute beds.
- Ellesmere Port Hospital: providing 60 beds as a rehabilitation, intermediate and outpatient facility.
- Tarporley War Memorial Hospital: a base for community services which serve the local rural population.
