About The Role
Provide specialized medical care for elderly patients. Perform comprehensive geriatric assessments and develop individualized treatment plans. Manage chronic conditions, coordinate healthcare services, and provide guidance on end-of-life care. Collaborate with healthcare professionals to optimize patient outcomes. Educate patients, families, and caregivers on geriatric health issues.
Responsibilities
- Provide specialized medical care for elderly patients.
- Perform comprehensive geriatric assessments and develop individualized treatment plans.
- Manage chronic conditions, conduct medication reviews, and coordinate healthcare services.
- Provide guidance on end-of-life care and advance care planning.
- Collaborate with other healthcare professionals to optimize patient outcomes.
- Educate patients, families, and caregivers on geriatric health issues.
Requirements
- Medical degree (MD or DO) and completion of a residency program.
- Board certification or eligibility in geriatric medicine.
- Proficiency in geriatric assessments, medication management, and palliative care.
- Excellent communication and interpersonal skills.
- Compassion and empathy for the unique needs of elderly patients.
- Ability to work collaboratively in a multidisciplinary team.
