Mental Health Practitioner

We are seeking a compassionate mental health professional to provide high-quality, specialist care to service users with complex needs. In this role, you will undertake comprehensive assessment, care planning, implementation and evaluation, using evidence-based formulations to develop person-centred packages of care that support recovery and reduce risk.
You will communicate complex and sensitive information with empathy and clarity, working closely with service users, carers, colleagues and partner agencies to ensure coordinated, respectful and inclusive support.
You will work within the Care Programme Approach (CPA), contribute to multidisciplinary and multi-agency working, and provide expert advice on service user needs.
You will maintain accurate clinical records across electronic systems, including RiO, PETs and safeguarding systems, ensuring compliance with Trust policies and professional standards.
The role also includes supervising junior staff and students, contributing to audits, research, training and service development, and supporting quality improvement.
You will help ensure services are clinically effective, responsive to the diverse needs of the community, and aligned with wider Trust goals and local mental health frameworks.
- Deliver high-quality nursing care as part of the multidisciplinary team.
- Act as duty worker to support co-ordination of the Slough Community Mental Health Team.
- Manage your own caseload as a key worker/care co-ordinator.
- Assess needs and agree goals using RiO, outcome measures and Wellness Recovery Action Plans.
- Arrange regular reviews and ensure care plans remain focused on recovery goals.
- Promote independence, social inclusion and recovery in the community.
- Support service users and carers to access benefits, welfare rights and appropriate services.
- Plan and deliver therapeutic, leisure and daily living interventions.
- Administer medication in line with legislation, Trust policy and professional standards.
- Maintain effective communication with service users, carers and partner agencies.
- Teach, coach and supervise junior staff within the CMHT.
- Use clinical skills such as motivational interviewing, cognitive behavioural approaches and phlebotomy where required.
