An exciting opportunity has arisen for a Band 4 Health and Social Care Coordinator (HSCC) to join our multidisciplinary team (MDT) within the Ashford locality. The HSCC plays a vital role in organising and coordinating weekly MDT hub meetings, bringing together professionals across health, social care, and voluntary services to support patients with complex needs.
This role is central to improving patient experience, reducing health inequalities, and delivering proactive, integrated care that helps prevent crises and avoid unnecessary hospital admissions. You will act as both a care coordinator and MDT facilitator, working closely with community nursing teams, GPs, and partner organisations.
The post is primarily based in Ashford, with travel required across the locality for home visits and hub meetings. Occasional support will also be needed within the Canterbury locality. We are looking for someone who demonstrates kindness, professionalism, responsiveness, and excellent communication, aligned to the Trust’s CARE Values into Action Framework.

As an HSCC, you will support the coordination and delivery of high-quality, person-centred care across the MDT. Key duties include:
- Coordinating MDT hub meetings, including processing referrals, gathering patient updates, arranging attendance, taking minutes, and following up on agreed actions.
- Carrying out telephone and face-to-face assessments to identify patient needs.
- Completing trusted assessor assessments (training provided) and supporting access to equipment and services.
- Signposting patients and families to appropriate health, social, or voluntary sector support.
- Maintaining clear and timely communication with the wider inter-professional team to ensure coordinated care delivery.
- Managing your workload flexibly and responding to changing service priorities with autonomy and professionalism.
