Care Needs Assessment, Support Planning and Activities Officer

Hours: 40 hours per week, Monday to Friday (between 8:00am and 6:00pm)

Some evening and weekend work may be required

Salary: £31,262.40 per annum

Work Location: Huddersfield Head Office

We are looking for someone with

  • Strong written communication skills and the ability to produce clear, structured documentation
  • Excellent organisational and time management skills
  • The ability to communicate effectively and sensitively
  • Initiative and sound decision-making skills
  • The ability to handle confidential information appropriately
  • Proficiency in Microsoft Office (Word, Excel, Outlook, PowerPoint)
  • A full UK driving licence and access to a car insured for business use

Role overview

Working with the Support Planning Manager, this role involves undertaking care needs assessments and working collaboratively with clients, families, professionals, and colleagues to develop person-centred support plans.

The role also includes organising and attending 6-weekly and annual service reviews to ensure clients achieve their maintenance and personal outcomes.

Additional responsibilities include:

  • Developing and maintaining a Menu of Wellbeing Enriching Activities
  • Supporting managers and teams to create tailored activity plans
  • Establishing an Activity Coordinator Champions Network
  • Coordinating group activities and events for all clients, including those attending the Day Centre
