STEPs team (short term enablement programme) need Community Reablement Officers that can demonstrate the 6 C’s.

Care - Compassion - Competence – Communication – Courage – Commitment.

You will support people in their own home for up to 6 weeks following a hospital stay or to prevent a hospital admission. You will assist and support people to live how they chose in a safe environment while promoting independence with practical support as agreed in their support plan, this is called reablement.

Responsibilities will include providing personal care, assisting people with mobility transfers following a movement & handling plan, providing support with medication, meals and understand safeguarding and risk assessments.

STEPs are regulated by the Care Quality Commission; you will understand how ways of working in the team makes sure we maintain the highest standard of care and excellence.

You will need a full Driving Licence and own transport to travel throughout the borough.
