Mental Health Practitioner

We are seeking to recruit a highly motivated and organised Registered Nurse, Occupational Therapist or Social worker to the Mental Health Out of Area team (secure)

The team consists of three practitioners who support CWP patients who are admitted into the secure pathway (High, Medium and Low). Each practitioner holds a caseload of patients and their role focuses on ensuring a strong connection and local link is maintained whilst these patients are in out of area hospitals. The team aim to build strong links with their MDT and families and support them along with their recovery journey in hospital and transition back into the community when ready for discharge.

The team is based at Bowmere hospital, however is a trust wide team and the post holder will be expected to travel across the North West footprint. The Forensic Support Service base is located in Saddlebridge and this could be considered as a base if required (please indicate at interview)

The team sits alongside the Forensic Support Service and links in with the team for peer supervision, training and clinical network activities.

The successful individual will need to demonstrate a knowledge of evidence based forensic risk assessment and formulation, to support pathway management, public protection and service user outcomes.

The post holder will require excellent communication skills and be able to work both autonomously and within a multi-disciplinary team.

A good knowledge base of the mental health act, secure inpatient provision, section 117 aftercare, ICB and local authority commissioning processes and mental health is required.
