Support Practitioner

We are currently seeking a skilled and experienced Support Practitioner to join our team. In this role, you will be responsible for providing a programme of person-centred support to customers referred into the Housing Related Support service in accordance with contract requirements.

Contract: Full time, 40 Hours per week ‍

* Key member of the North East Lincs HRS Team.

* To provide a programme of person-centred support to customers referred into the HRS Intensive Rough Sleeper Support Service in accordance with contract requirements.

* To engage with customers on a weekly basis to support individuals who have a history of rough sleeping or multiple and complex needs and are at risk of rough sleeping, referred into the service, with high-quality structured support. The objective is to assist them in making the transition from a chaotic street lifestyle to sustaining stable accommodation.

* The co-provision of support/resettlement plans, risk management plans, crisis intervention, and engagement with customers in order to deliver positive outcomes in a service that offers involvement and empowerment for customers.

* To provide a high-quality service, ensuring outstanding customer service and service delivery by acting as an ambassador for Humbercare and a point of contact for customers, working in conjunction with the Homelessness Prevention Service and other voluntary and statutory services such as Rough

* Sleeper Initiative-funded partners, mental health services, social care, and accommodation and support providers.

‍Hours will be delivered between core office hours and early evening.

* Educated to Level 2 Diploma or equivalent.

* Experience of working in a social care or similar environment

* Knowledge of varying Complex Needs and their impact on the ability for a person to access housing or maintain independent living.

* Knowledge of local statutory and voluntary support agencies

* Working knowledge of person-centred, strengths-based approaches to support and care planning.

* Working experience of dealing with complex service users/customers/public

* Experience of working with others to achieve successful outcomes.

* Excellent communication, interpersonal and problem-solving skills.

* Team working experience, working cooperatively with colleagues in complex, pressurised situations.

* Excellent organisational and time management skills and the ability to effectively manage a caseload.

* Willing to work towards a recognised housing/care/support qualification.

Salary: £27,040.00 per annum - 40 hours per week
