The Role
This role does not meet the eligibility criteria for a or a NHS Greater Glasgow and Clyde (NHSGGC) is unable to provide candidates or employees with a Certificate of Sponsorship (COS).
This is an opportunity to become a part of a large multidisciplinary skilled and experienced community mental health team.
You would be responsible for contributing to the care and support of people referred into the service. The post holder would be expected to provide care and supports as part of an agreed plan of care which involves the use of a strength-based recovery focussed approach to care which proactively involves the person and optimises self- management.
There would be the expectation that you are able to undertake duties without the direct supervision working in partnership with patients, carers and other professionals/agencies in line with legislative frameworks agreed standards and clinical policies. This would involve working in a variety of home/near to home community settings.
The post holder would be expected to participate in caseload line management supervision and clinical supervision which are critical aspects of the role. NHSGGC (2016) Community Mental Health Team (CMHT) Operational Framework defines the CMHT as forming part of a whole system approach to mental health services for adult and older adult population. The service is delivered in conjunction with Primary Care Mental Health Team PCMHT; Acute services (Crisis/Home treatment services and Out of Hours) Specialist Mental Health services and a range of statuary and non-statutory services that support the delivery of care.
CMHTs function using an integrated multi-disciplinary approach and aims to: Work with people with complex mental health problems and associated risks who typically require longer term care and treatment multidisciplinary engagement and with the capacity to provide assertive care and to manage higher levels of risk. Work in partnership with patients, families and carers, primary care and other agencies to where needed, to support people with complex mental health needs.
Deliver services in a suitable environment that is accessible to the individual (and their carers). Thais will include team base, GP surgery, the patient’s home or other appropriate community settings. Interventions delivered by the CMHT will be on a “stepped/matched care” basis, based on the individualised assessment of need and will be governed by the principle of the delivery of the minimum effective level of intervention.
All patients being cared for have a named nurse or key worker who is tasked with working closely with the identified patients/carers to develop a participative therapeutic relationship and deliver high quality mental health care in an environment where patients feel valued and where confidentiality is respected.
Driving licence is not essential but is desirable.
Duration, Location, and Working Pattern:
- Location based in Riverside Resource Centre
- Permanent Contract
- Full-time Position (36 hours per week)
- Working hours for this post are Monday to Friday
Key Responsibilities
- Ability to communicate sensitive and/or confidential information within agreed policy frameworks
- Ability to form therapeutic relationships, understanding the implications of their own value base in relation to others.
- Ability to listen, support and signpost people presenting with distress while lone working until reviewed by senior staff.
- Ability to work in anti-oppressive and anti-discriminatory manner.
- Ability to carry out delegated tasks as directed and not always under direct supervision due to lone working criteria
- To deliver interventions as defined in care plans in collaboration with keyworker and other services
- To collect, transport and monitor compliance with medication as identified in care plan
- To deliver high quality interventions that take into account age appropriate needs as identified in care plans
- To contribute and collaborate to specialist shared assessment, care assessments and care plans as required

Essential
- Working knowledge of mental health nursing care
- Working knowledge of mental health issues
- Evidence of using digital technology/ information systems/ email and basic applications
- Evidence of skills and behaviours that demonstrate effective written and verbal communication
- Evidence of skills and behaviours that demonstrate application of HCSW Standards and Code of Conduct
- Ability to form working relationships with others
- Ability to follow instructions and undertake work without direct supervision
- Full driving licence required
Desirable
- Educated to or actively working towards SVQ3/ SCQF level 6 or equivalent experience
- Completion of a health / mental health / social care programme or equivalent experience
- Knowledge of health and social care issues that impact on mental health and wellbeing
- Experience of working in the CMHT environment
