The Frailty Unit is an established, dynamic service delivering specialist, person-centred care to frail and acutely unwell patients with complex health and social care needs. We are seeking motivated professionals to join a multidisciplinary team committed to high-quality care and excellent patient outcomes.
About The Role
You will contribute to comprehensive geriatric assessment, early intervention, and safe, effective discharge planning, supporting smooth patient flow across the hospital.
The role also offers the opportunity to be involved in the ongoing development of our Same Day Emergency Care (SDEC) service, enhancing timely assessment, treatment, and safe discharge for frail patients.

Provide 24-hour leadership and accountability for frailty clinical areas, ensuring safe, effective, and compassionate care for older people, ensure continuity of care across all shifts, including escalation of concerns and coordination with on-call teams.
- Promote patients, families, and carers as active partners in care, recognising the complexity of frailty, multi-morbidity, and personalised care planning. Support shared decision-making, effective communication, and respect for individual, cultural, and end-of-life needs.
- Develop and support nursing staff to build frailty-specific skills, including comprehensive geriatric assessment, delirium recognition, falls prevention, dementia, and end-of-life care.
- Lead effective, timely, and person-centred discharge planning from admission, working closely with patients, carers, community services.
- Promote strong multidisciplinary working across acute, community, therapy, social care, and specialist frailty teams .
- Provide expert advice and leadership in frailty care, supporting complex decision-making, escalation planning, and staff wellbeing.
- Monitor and maintain high standards of cleanliness, safety, and the care environment, ensuring it is suitable for frail and vulnerable patients.
- Ensure compliance with health and safety, infection control, medicines management, and equipment standards.
- Ensure safe staffing and skill mix .
