Community Mental Health Practitioner

We are seeking either a Registered Mental Health Nurse, Occupational Therapist or Social Worker to work within the multidisciplinary team with Mental Health Together.

We have two part time roles available.

Mental Health Together is part of the Community Mental Health Framework.

The Community Mental Health Practitioner will have responsibility to assess, plan, coordinate, deliver and evaluate care for 18yrs+ adults in the area of South West Kent, experiencing mental health disorders.

Mental Health Together functions within a community setting, working with individuals who have identified recovery needs as a result of mental health issues using evidenced based, client centred, hope and recovery, health promotion, relapse prevention and orientated principles and practice in a socially inclusive manner. We actively work in collaboration with family, networks and providers or mental health support in the community.

  • Carry out and coordinate comprehensive, systematic mental health assessments which take account of relevant mental, physical, social, cultural, psychological, spiritual, and environmental factors, in partnership with service user and others through interaction, observation and measurement.
  • Work collaboratively with other disciplines, service user and their carers to agree a holistic person-centred care plan that addresses the needs identified through assessment.
  • To deliver and evaluate safe, person centred care in partnership with the service user and their carers to support recovery.
  • To provide educational support to optimise health and wellbeing; promoting self-care where possible, helping people to make choices about their needs and involve their carers where appropriate.

Our teams support children, young people and adults with a wide range of mental health needs. We also provide specialist services, including all-age eating disorder services and neurodevelopmental assessments for children and young people, such as autism and ADHD.
