Every day, thousands of unpaid carers across County Durham provide vital support to family members, friends and neighbours. At Durham County Carers Support (DCCS), we are committed to ensuring those carers receive the information, advice and support they need to maintain their own health, wellbeing and quality of life.
We are looking for a compassionate, motivated and organised individual to join our team as a Carer Support Co-ordinator.
Hours of work: 21 hours per week
Salary: £26,439 per annum pro rata (£15,863 actual for 21 hours)
About the Role
This is a rewarding and varied role where no two days are the same. You will work directly with unpaid carers, providing practical advice, emotional support and guidance to help them navigate the challenges of their caring role.
You will:
- Provide information, advice and support to carers by telephone, online and face-to-face.
- Carry out carer registrations and support assessments.
- Help carers access welfare benefits, grants and financial support.
- Support carers to apply for NHS Carer Break funding and emergency planning services.
- Signpost and refer carers to relevant organisations and services.
- Build relationships with GP practices, community organisations and partner agencies to identify and support hidden carers.
- Manage your own caseload and maintain accurate records.
- Attend community events and awareness activities across County Durham.
About You
We are looking for someone who:
- Understands the challenges faced by unpaid carers.
- Has experience supporting vulnerable adults or providing advice and guidance.
- Has knowledge of welfare benefits, grants or support services.
- Can build positive relationships with carers, professionals and partner organisations.
- Has excellent communication and organisational skills.
- Is able to work independently and as part of a supportive team.
- Is confident using IT systems and maintaining accurate records.
