Location: Hybrid – Home working, community venues, GP Practices and remote delivery
Hours: 15 hours per week
Contract: 12 month fixed term
The Diabetes and You Clinical Programme Coordinator is a clinically informed education and programme delivery role supporting the operational management, development and delivery of the Diabetes and You structured education programme across Cornwall and the Isles of Scilly.
The Role Combines

Main duties of the job
Requirements
The post-holder will provide clinical insight and operational leadership to support safe, effective and sustainable delivery of Diabetes and You aligned to NICE structured education standards and QISMET requirements. The role will support delivery of:
- patient structured education courses,
- educator forums and mentorship,
- Train the Trainer activity,
- programme reporting and evaluation,
- educator coordination,
- and ongoing development of educational content and resources.
The post-holder will work closely with the Education Lead, educators, Primary Care Networks (PCNs), practices and wider system partners to improve access, quality and consistency of structured diabetes education across Cornwall and the Isles of Scilly.
The role combines clinical facilitation, workforce development and programme leadership, requiring the ability to work flexibly across education delivery and operational coordination responsibilities.
