This is a full-time, permanent role with hybrid working, and a salary of £38,192 to £42,012 per annum plus an annual retention bonus of £3,000 subject to eligibility.

This is an excellent opportunity for an enthusiastic Social Worker to join our Community Team for People with Learning Disability.

The Role

You will be working as a Social Worker to support young people in transition from Children's to Adult Learning Disability Services.

Your Role Will Involve

  • Located within a multi-disciplinary community team you would be supporting young people with learning disabilities and their families and carers to help ensure a positive transfer to adult services, including liaising with Children's services and any other involved agencies.
  • You would be involved in carrying out assessments, undertaking care reviews and supporting care planning.
  • You would take part in a duty rota to deal with incidents, safeguarding alerts and information requests.
  • You would work as a Social Worker and Health Professionals to protect and promote the interests of vulnerable people.
  • Undertake safeguarding investigations and capacity assessments and best interest decisions relating to adults with a learning disability.

What We Are Looking For

  • A qualified Social Worker with a qualification such as a BA, BSC Diploma or equivalent.
  • Current registration with Social Work England.
  • Experience of working in a health and social care environment.
  • Good understanding of safeguarding protocols.
  • Knowledge of relevant legislation.
  • Committed to working co-operatively as part of a team.
