Job Overview
An exciting opportunity has arisen in Hywel Dda University Health Board to join our Acute Medical Rehabilitation team. This is an innovative focused team, in place to support the recovery and rehabilitation of patients across the medical wards . The post will involve working with patients on medical wards at Withybush Hospital, including cardiac, respiratory, endocrine and gastroenterology, surgical, admission units and ITU.
The post holder will be working as part of a multi-disciplinary team including Physiotherapists, Occupational Therapi sts, Social Workers, Discharge Liaison Nurses, Nurses and Doctors.
Applicants will need to be highly motivated with a keen drive to achieve patient centred, holistic, and compassionate care. With a wide variety of patients and conditions seen within acute medical services, and through working alongside a wide array of multidisciplinary staff this post brings with it a wealth of learning opportunities. Due to the high pace of the environments, applicants will need to be adaptable and able to work in busy and pressurised environments.
Main duties of the job
Duties will include: supporting and completing holistic patient assessments, with consideration of rehabilitation needs and strategies/options to aid a safe and timely discharge. To facilitate continuity of patient care.
The post holder will play a pivotal role in delivering rehabilitation to patients admitted to the medical wards to promote independence. This could include bed, chair, and standing activities, gait re-education, and gym based exercise programmes.
You will be expected to support patients alone at times, though all work undertaken will be under the guidance of qualified Physiotherapists. Recognition of your scope of practice is of paramount importance to recognise when qualified support is required.
As the service is working towards providing a seven day service, please note candidates will required to support a seven-day rota as and when this comes into place.
The University Health Board proactively supports training and development both as part of internal and external CPD and you will be expected to complete appropriate mandatory training.
