About The Role

An exciting opportunity has arisen to join the specialist palliative care team as a Clinical Nurse Specialist. The service spans across hospital and community inclusive of Rosewood Oncology and Palliative Care centre at Dewsbury District Hospital. Main duties

To provide highly specialised complex palliative care advice and support to patients with a diagnosis of cancer or other life limiting illness, and their carers within both the acute and community setting. To maintain and improve standards of care and promote a seamless service within a multi-professional framework. This will be achieved through clinical practice, management, education, research and audit of professional activities, taking into account objectives of the clinical service group, corporate objectives and within the Clinical Governance Framework.

Key Requirements

  • Organises, prioritises and evaluates workload based on accepted referral criteria and discharge criteria in line with operational protocols.
  • Act as a role model and clinical expert within Specialist Palliative Care, both within the Trust and externally.
  • Facilitate clinical care by providing expert assessment. Determine priorities of care and advise on symptom control, emotional, psychological, spiritual and social care.
  • Contribute towards a multi-professional approach in the management of patients within Specialist Palliative Care, especially in relation to ongoing care needs.
  • Work collaboratively with primary care colleagues through the implementation of and attendance at Gold Standards Framework meetings, other associated forums .and shared governance initiatives.
  • Work in partnership with other health care and Specialist Palliative Care providers such as care homes and hospices, referring appropriately and facilitating a seamless service for patients.
  • Establish and maintain effective communication with patients and carers/relatives.
  • Demonstrate knowledge in relation to pattern of disease, markers of disease progression and understand the range of treatment available.
  • Demonstrate knowledge of drugs used for the control of symptoms.
  • Develop own ‘expert’ clinical knowledge and participate in the development of others' clinical knowledge.
  • Undertake clinical supervision of nursing colleagues on an individual or group basis.
  • Provide appropriate support and advice to bereaved carers known to the Specialist Palliative Care service and to refer on to local bereavement services as appropriate.
  • Act as a core team member of the Specialist Palliative Care MDT.
  • Recognise ethical and legal issues which have implications for nursing practice and take appropriate actions.
  • Promote the use of nationally recognised end of life care tools to improve the quality of care.
  • Continually monitor standards of care and participate in the improvement of care through benchmarking, clinical audit and research.
  • Evaluate and disseminate research findings and facilitate evidence-based practice.
  • Will have responsibility for supporting the lead nurse in leading identified and agreed aspects of the service, in the development of care systems, protocols and guidelines and working as part of a multi-disciplinary team with a focus on quality improvement.
