Fantastic opportunity for a Registered Health Professional qualified in Physical Health Assessment and Non-Medical Prescribing to join our Acute Visiting Service (AVS), part of the collaborative service provided by Acute Care at Home across Stoke on Trent and Staffordshire Moorlands. You will be an experienced clinician who will be acting within your professional boundaries and competencies.

The 3 aspects of Acute Care at Home are CRIS 2hr Urgent Community Response, Virtual Wards and Acute Visiting Service. This post will be expected to support all 3 aspects of the service but be predominantly available to respond to AVS referrals from Primary Care, to support patients within their own homes who are unable to attend a GP appointment or need same day intervention to reduce the risk of hospital attendance. The AVS runs Monday-Friday from 13:00hrs-20:00hrs, but the role will also support all patients across the Acute Care at Home seven days a week from 08:00hrs-22:00hrs.

We are keen to meet dynamic, enthusiastic and highly motivated Registered Healthcare Professionals with experience in urgent or primary care, interested in working in our supportive admission avoidance team providing the highest standards of care to keep our patients safe in their own homes.

This exciting role will provide a high-quality service to housebound patients on behalf of their registered GP Practice, and the wider PCN referred to our AVS.

The Clinical Practitioner will review and support patients in their own home, ensuring robust medical management plans are put in place to safely keep patients at home and avoid admission to hospital.

You will be an experienced clinician who will be acting within their professional boundaries and competencies. You will demonstrate safe, critical thinking in the clinical decision-making process, providing care for patients in their own home from initial history taking, clinical assessment, diagnosis, treatment, referral (where required), evaluation, and follow up of care.

Our focus is reduction of ED attendances and safe admission avoidance, acute/subacute interventions to improve outcomes for our patients across ACAH.

Applicants will need to hold a full driving licence and have access to a vehicle for work, as this is required for the role.

  • Assessing, diagnosing and treating patients referred into the Acute Visiting Service (AVS) Monday – Friday covering the referral hours of 12 – 8pm
  • Working closely with the overall Acute Care at Home Service and Virtual Ward Team to assess and manage complex patients with acute care needs.
  • Supporting, where needed and appropriate, with the Community Rapid Intervention Service 2hr Urgent Community Response covering the hours of operation 7 days a week 8am – 10pm
