About The Role
This role offers the opportunity to join a highly regarded pharmacy team providing expert pharmaceutical care to patients requiring early medical management at SJH and also work at the interface between primary and secondary care in community hospitals.
Acute and General Medicine at St John’s Hospital delivers care to over 100 patients per day, working across: acute assessment areas, the Medical High Dependency Unit and general medicine inpatient wards. As part of this team, you will:

- Work alongside the wider multidisciplinary team.
- Contribute to patient flow and site capacity management, ensuring timely and effective pharmaceutical interventions.
- Provide expert medicines management, optimising prescribing decisions to enhance patient safety and outcomes.
West Lothian Community Hospitals Care For Patients Requiring Ongoing Hospital-based Complex Clinical Care. Working At The Interface As Patients Move From Acute Care At SJH To WLCH You Will:
- Help frail patients achieve the best from their medicines.
- Work collaboratively with medical, nursing and AHP colleagues in delivering patient-centred care and realistic medicine.

We are looking for a highly motivated and enthusiastic pharmacist with a passion for delivering exceptional clinical care.
Essential Qualifications And Experience:
- A Master of Pharmacy Degree (or equivalent).
- Registration with the General Pharmaceutical Council (GPhC).
- Completed training or equivalent competence mapped to the
- Being qualified as an independent prescriber would be desirable although training opportunities are available for post-graduate study.
- Experience in proactively influencing prescribing, using clinical knowledge to optimise patient care.
- Excellent communication and problem-solving skills, enabling effective decision-making in an acute medical setting.
- A strong patient-focused approach, with the ability to work effectively both autonomously and within a multidisciplinary team.
- A proactive attitude, with a commitment to continuous learning and professional development.
