Job Overview
As part of the wider multi-disciplinary team at Forward Leeds, you will support individuals affected by alcohol and substance misuse in preparing for residential rehabilitation placements outside the local area. This includes providing one-to-one support, facilitating a weekly rehab preparation group, and delivering structured interventions to help service users get ready for admission.
You will play a key role in the referral process completing all necessary paperwork relating to funding, finances, and data tracking systems. Your duties will also include transporting service users to and from out-of-area rehab facilities for assessments, admissions, and discharges, using one of our company pool cars.
Collaboration is central to this role. You will work closely with case managers, other professionals, and rehab providers to ensure that each service user receives person-centred, coordinated care throughout their treatment journey.

Main duties of the job

Included in the role will be visits to these facilities using your own vehicle and transporting service users for their assessments, admissions, and discharges. You will also develop and facilitate a weekly rehab prep group and deliver interventions and sessions based on preparing services users for admission. You will link in and work alongside case managers to ensure person centred treatment, liaise with other professionals and rehab staff. You will be responsible for completing the appropriate paperwork for referrals, funding and finances including keeping up to date with data tracking and systems.

- Carry out assessments, develop and review care plans and keep case files up to date in accordance with policies and procedures.
- Deliver one to one structured session within a care planned approach using appropriate screening tools
- Plan and facilitate group sessions for service users who are preparing for admission to an out of area rehab facility. This will include the development of new material and planning for sessions.
- Work within Adult Social Care commissioning guidelines
- Maintain professional working boundaries with service users while always maintaining a non-judgemental approach
- Carry out drug and alcohol testing as required
- Attend various multi-agency meetings and forums, representing the service user and service partnerships.
- Be an essential car user with full driving licence and the willingness to transport/escort service users to detox and rehab facilities using own transport.
- To work flexibly across all sites to meet the business need, with the expectation to cover some evening work on a rota basis. This would be 11.00 till 19.00.
- To work within established care pathways and referral procedures set out by Forward Leeds including partner agencies.
- Responsible for maximising opportunities for client participation and co-production.
- Responsible for maintaining accurate and appropriate records in accordance with the organisations confidentiality policy and to be able to provide information and reports when requested.
- Ensuring that clients’ rights are respected and that their views and wishes are adequately represented in the care planning and support process.
- Ensure that safeguarding concerns are acted on in line with our policies and procedures by understanding and adhering to the principles of Safeguarding Children and Vulnerable Adults.
- Taking responsibility for your own personal development and training needs, based on the service objective
- To seek out and explore areas of development for the service and improving the care provided to our service users.
- To participate in formal supervisions, peer supervisions and practice reflection sessions.
- Understanding the importance of equality and diversity in the workplace and taking responsibility for implementing it.
- Adhere to the principles of Information Governance, ensuring the security and safety of premises, equipment, records, and resources.
- Work within the Policies and Procedures of St Annes Community Services and Forward Leeds; and to National standards of NICE, CQC and other governing bodies.

Experience
Desirable criteria
- At least 2 years experience in Social Care
- Full UK Driving License with access to a car with business insurance
