Salary: £25,027.20 per annum (Pro Rata)
Based at: To be agreed (Wrexham/South Denbighshire)
Role Purpose
Under the direction of their line manager, the post holder will undertake support planning, provide one-to-one, group support and interventions to improve the lives of unpaid Carers, families and friends affected by serious mental illness and or Substance misuse issues.
To equip unpaid Carers, families and friends with skills, knowledge, and coping strategies to ensure their own well-being is improved and maintained. This is to be achieved by visiting clients within their own homes, in the community, at hospital settings and or across groups established across, Wrexham and the South Denbighshire area, thus enabling the unpaid Carers to manage the impact of their caring role.
To work in partnership with other statutory, voluntary, private sector organisations and community facilities to ensure everyone can access other existing services.

Rhoi sgiliau, gwybodaeth a strategaethau ymdopi i Ofalwyr di-dâl, eu teuluoedd a’u ffrindiau er mwyn sicrhau bod eu llesiant eu hunain yn cael ei wella a’i gynnal. Mae hyn i'w gyflawni trwy ymweld â chleientiaid yn eu cartrefi eu hunain, yn y gymuned, mewn ysbytai a/neu ar draws grwpiau a sefydlwyd ar draws ardal Wrecsam a De Sir Ddinbych, gan alluogi'r Gofalwyr di-dâl i reoli effaith eu rôl ofalu.
Gweithio mewn partneriaeth â sefydliadau statudol, gwirfoddol, sector preifat a chyfleusterau cymunedol i sicrhau bod pawb yn gallu cael mynediad at wasanaethau eraill sy'n bodoli.
