- In depth knowledge of person centred, asset based support planning and risk assessments including knowledge of safeguarding children and adults.
- Awareness of factors that put people at risk of homelessness
- Awareness of factors that put people at risk of hospital admission and / or delayed discharge
- Skilled at IT and updating case management systems
- Experience of working with a diverse range of people and working effectively in partnership with others
- Level 3 NVQ qualification or equivalent work level experience.
- Be person centred at all times and work with people to identify what is important to them and ensure the most appropriate intervention provides an effective route to support.
- Deliver a ‘no wrong door’ approach, providing a supported referral into other services where more appropriate
- Work directly with the Social Reablement team, allocating cases effectively to the most appropriate intervention
- Work collaboratively with statutory and non-statutory services including but not limited to Hospitals, CMHTs, Substance Misuse Teams, Healthcare professionals, social prescribing initiatives, early help and preventative approaches of partner agencies, providing a coordinated and holistic service involving advocacy and crisis response.
